  1. Medical License/Certificate of Doctor/Physician in charge
  2. Passport sized photo of Doctor/Physician in Charge
  3. Business registration document of clinic
  4. Accreditation certificate issued by governing body in your country/state
  5. Proof of insurance (for UK and USA clinics)
